Facility Evaluation Report

Licensing Program Analyst (LPA) Jason Lund arrived unannounced to complete a case management visit. LPA Lund met with Assistant Administrator Lionel Bazan and explained the purpose of today's visit.

On May, 29,2024 Department of Social Services/Community Care Licensing complied the requirement to exclude Staff (S1) from working at licensed facilities. LPA Lund interviewed Assistant Administrator Lionel Bazan who stated that S1 has not worked at the facility in the last five years.

No deficiencies were observed during today visit. Exit interview done and a copy of the report, was left.
