The inspector’s account

On 03/27/2025, Licensing Program Analysts (LPAs) Arielle Pascua and Triel Lindstrom arrived unannounced to this facility to conduct an annual visit. LPAs met with Facility Designated Administrator (FDA), Maricel Casillas. One other staff member was present, Oliver Mengito.

This facility is licensed to served and retain up to 4 residents, of which 3 may be ambulatory with 1 resident who can be non-ambulatory residing in room #4. This facility is also vendorized by Valley Mountain Regional Center to accept and retain Level 4I residents at this time.

Current census was 3. 1 out 3 residents were out at their respective day program at this time. A brief interview with FDA Casillas was conducted.

A review of 1 resident files and 2 staff files were conducted. All resident and staff files were complete and up to date. The facility designated Administrator, Maricel Casillas, did have her Administrator certificate posted with an expiration date of #7018782735 and expires at 07/16/2025 and in compliance at this time.

Tour of the facility was conducted. Smoke alarms and carbon monoxide detectors were tested and observed to be in good repair at this time.

Common areas were toured. Dining area, living area, and all other areas intended for resident use were observed to be furnished and maintained in compliance at this time.

Kitchen area was toured. Cabinets and drawers were reviewed. Knives and toxins were observed to be made inaccessible at this time. It was observed that the facility has a 2 day perishable and 7 day non-perishable food supply.

Fire extinguisher, hanging on kitchen wall, was observed to have been annually serviced by the Local Fire Company, R. Brown and Associates on 02/14/2025 and is in compliance at this time.

Cabinet below the sink was reviewed. It was observed to contain cleaning agents and supplies which were locked and made inaccessible at this time.
