Facility Evaluation Report

On 08/28/2025, Regional Manager (RM), Stephenie Doub and Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit.

The purpose of this visit was to follow up on the Licensee’s intent regarding the continued operation of the facility.

Upon arrival, RM Doub and LPA Pascua were greeted by Licensee Consuela Farcas. The purpose of the visit was explained, which was to follow up on an unannounced annual inspection conducted on 07/16/2025, during which the Department was unable to gain entry.

During the greeting, Licensee Farcas stated that she intends to forfeit her license. LPA Pascua confirmed this statement, and Licensee Farcas affirmed her intention to surrender the license. LPA Pascua requested that Licensee Farcas submit a written notice of her decision to the Department as soon as possible.

There are no deficiencies observed during this visit.
