Facility Evaluation Report
On 09/30/2025, Licensing Program Analyst (LP) Renee Campbell conducted an unannounced annual inspection at Howard Prep. LPA Campbell identified themselves and explained the purpose of the visit and asked to speak to the Administrator. LPA Campbell met with Brandie Montes, Administrator. LPA Campbell selected resident and staff files for review. The Day Program is licensed to serve 290 ambulatory and 30 ambulatory adult clients over the age of 18 who are developmentally disabled. There is a currently a census of 35 clients enrolled in the program.
LPA Campbell toured the facility with Administrator Montes. The staff break room and kitchen was clean and sanitary. Staff were observed assisting residents with activities of daily living and socializing. The water temperature was measured at 99.8 degrees which is below the required regulation of 105 to 120 degrees Fahrenheit. During water measurement, the water rose to a high level while LPA Campbell was waiting for the water to get hot. The sink then began to leak. The Administrator called for staff to clean the water and put a "Wet Floor" sign over the puddle. Administrator agreed to turn off the sink and put up a sign until it is repaired
The thermostat was set at 78 degrees which is within between 68 degrees F (26 degrees C) and 85 degrees F (30 degrees C). First aid kits were observed containing a first aid manual, band aids, tweezers, scissors and thermometer.
The fire extinguisher was obs erved to have been last inspected on 03/11/2025 and was fully charged. During the months of October, November, February and March, fire drills were conducted successfully.
LPA Campbell reviewed 4 resident files and 2 staff records during the visit. Both staff were found to be finger print cleared as required.
