The inspector’s account

On 11/19/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the community to conduct an annual inspection visit. LPA Campbell met with Cami Azevedo, Administrator and explained the purpose of the visit. The community is licensed to service 4 developmentally disabled ambulatory adults ages 18 to 59. Two of the residents may be non-ambulatory in bedrooms 3 and 4. The community is housed in a single story building with 4 bedrooms and 2 bathrooms.

Upon entry, LPA Campbell observed 1 resident watching TV and another resident on the phone speaking to their parent. An additional resident was in his room. One other resident was at day program. See Something Say Something sign, the facility sketch, and Emergency Disaster Plan was observed on the wall. There was also a Menu for the month and an Emergency Disaster Kit Inventory List posted along with an Emergency Disaster Plan.

In the kitchen, LPA Campbell measured the hot water at 114 degrees Fahrenheit. There are 2 days of perishable foods and 7 days of non-perishable foods observed. The refrigerator temperature was 37 degrees Fahrenheit and the freezer was kept at 0 degrees Fahrenheit. A container with fresh fruit was displayed on the counter. The bathroom temperature was measured at 112 degrees Fahrenheit. During a tour of the community, LPA Campbell observed bedrooms with night stands, lamps, beds and closets. There is one handicap accessible bathroom for non-ambulatory residents.
