Licensing Program Analyst (LPA) Jason Lund arrived announced to conduct an case management visit. LPA Lund was met with Sidney Vessel, who allowed entrance into the facility.
A brief interview was conducted with Sidney Vessel. LPA Lund toured the facility and confirmed that there is no residents in care. As of 5/7/2025 the facility has no residents in care and is closing the facility.
