Facility Evaluation Report
On 04/04/2025, Licensing Program Analysts (LPA) Arielle Pascua and Triel Lindstrom arrived unannounced to this facility to conduct a Health and Safety Visit. LPAs met with Facility Designated Representative, Venice Andrews, and explained the purpose of the visit.
The purpose of the visit was to conduct a Health and Safety visit in response to a fire that occurred at another facility and had to move 2 residents to this location at this time.
Current census was 6. A brief interview with FDR, Andrews, was conducted.
The LPA continued with facility visit to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.
There were currently two staff members on site. LPAs toured 4 resident bedrooms. Clothing, bedding, and other furnishings were sufficient to meet the residents needs at this time.
Medication were also reviewed to ensure medication was transferred and was provided in a timely manner.
5 out of 6 residents were observed in the dining area eating. 1 out 6 residents were sleeping in their respective bedrooms.
LPAs observed no immediate issues to the health and safety to the residents at this time.
The department will continue to monitor the situation with health and safety checks at this time.
Based on the observations made during this visit, there are no deficiencies cited during this visit. An exit interview was conducted and a copy of this report was provided to the facility at the end of this visit.
