Facility Evaluation Report
On 04/04/2025, Licensing Program Analysts (LPAs) Arielle Pascua and Triel Lindstrom arrived unannounced to this facility to conduct an annual visit. LPAs met with Facility Designated Administrator (FDA), Kenroy Anderson and explained the purpose of the visit.
It was learned on 04/03/2025 via email that the facility had a fire in the garage of the facility and residents were no longer residing at the facility and have been moved to other facilities at this time. LPAs conducted a tour of the facility to ensure that there were no residents in care.
LPAs observed that there are currently no residents in care at this time. LPAs conducted a tour of 3 resident bedrooms. Bedroom 1 did not have a bed in the room at this time or belongings at this time.
Bedroom 2 had two resident beds, one of which was a hospital bed. LPA Pascua observed resident belongings in the chest of drawers however, it was learned that this resident was out at the hospital at this time. Bedroom 3 had 2 resident beds with belongings such as incontinence supplies, pajamas, clothing, and pictures.
Living areas, dining areas, and other areas intended were toured. A tour of the garage was conducted. LPAs observed black soot throughout the garage area. It was stated that the fire department has been at the facility and the fire is undergoing investigation.
LPAs reviewed 4 resident files. A resident files were incomplete at this time.
The following deficiencies were cited on the following LIC 809-D per Title 22, Division 6 of the California Code of Regulations.
An exit interview was conducted with the facility representative Kenroy Anderson and a copy of this report, along with appeal rights, was provided.
