Facility Evaluation Report

Unannounced case management visit made out to this facility on 11/200/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility staff person, Dwight Francis, who was briefly interviewed at this time.

This LPA requested that the facility staff person go ahead and contact the facility designated Administrator to inform them that CCL was present at this time.

The facility designated Administrator, Venice Andrews, arrived later to this facility. A brief discussion was held with the facility designated Administrator at this time.

The purpose of this visit was explained to the facility designated Administrator at this time. This LPA stated that the nature of this visit was to inform this facility, and it's representative, that the person, Kenroy Anderson, was no longer fingerprint cleared and had been excluded from being employed or even be present in any licensed care facility through this Department at this time.

A copy of the Exclusion letter was presented and given to the facility designated Administrator at this time.
