Facility Evaluation Report

Licensing Program Analyst (LPA) Renee Campbell arrived announced to conduct a closure visit. LPA Campbell met with licensee Irene Garde and explained the purpose of the visit.

Garde said that there have been zero clients at the facility since March 3, 2025. LPA Campbell was notified by the former licensee that the facility would be closed due to retirement and a need to take care of her family. LPA Campbell toured the facility and confirmed that there are no clients present. Currently, new non-resident tenants/family members are residing in the home.

This facility is hereby closed, effective 05/28/2025. LPA Campbell retrieved Garde's license. An exit interview was held and a copy of this report was left with Garde.
