Facility Evaluation Report

On 2/09/2023 at 10:00am, Licensing Program Manager (LPM), Stephen Richardson, Licensing Program Analyst (LPA) Arielle Pascua, and Valley Mountain Regional Center Liaison (VMRCL), Jennifer Stone arrived at this facility unannounced to conduct a case management visit. LPM, LPA, AND VMRCL were all greeted by staff member (SM), Alma Moyamoy and explained the purpose of the visit. At this time it was asked for SM Moyamoy to contact the Facility Designated Administrator (FDA), Julian Hawes to inform him that CCL and Valley Mountain Regional Center were at the facility at this time. It was at this time that FDA Hawes notified LPM Richardson that he was out of the facility at this time to drop the resident off at their respective day program. FDA Hawes informed LPM Richardson that he will be arriving back at the facility in 30 minutes. LPM, LPA, AND VMRCL waited 30 minutes for FDA Hawes arrival outside of the facility. At 10:30am, LPM, LPA and VMRCL went back to the facility and asked for SM Moyamoy to call FDA Hawes. FDA Hawes allowed CCL and VMRC to enter the facility while he was not there. Current census was 4.

During this time it was learned that the SM did not have facility files readily available for CCL and VMRC to review because it was stated by SM Moyamoy that she did not have the key to the facility files and medication. At 10:45am, LPM, LPA and VMRCL toured the facility.

At 11:30am, FDA Hawes and SM Iboni James arrived at the facility. A brief interview with FDA Hawes was conducted. CCL and VMRC asked FDA Hawes for facility files and were able to look at facility files at this time. LPA Pascua reviewed facility documents and obtained copies.

Due to the observations made during this visit, The following deficiency was cited per California Code of Regulations, Title 22, Division 6, Chapter 8.

An exit interview was conducted, a copy of the 809, 809-D and appeals rights were provided via email to the Facility Designated Administrator.
