Facility Evaluation Report
On 11/10/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA was met by Nicolette Taylor and explained the purpose of the visit.

LPA Pascua handed the Order to Licensee/Facility of Immediate Exclusion From Facility letter to Nicolette Taylor and explained that staff (S1) is excluded from any involvement in the facility effective immediately. Facility staff (S1) is to be removed from all shifts and disassociated from the facility in Guardian.
No citations were issued on today's date. A copy of this report will be emailed to the Facility Administrator at the end of this visit.
