Facility Evaluation Report

A Non-Compliance Conference (NCC) was conducted on this day, 04/22/2026 by the Sacramento South Regional Office via Microsoft Teams. The purpose of this NCC meeting was to discuss the deficiencies and incidents within the last 12 months. Present at the meeting were Regional Manager (RM), Stephenie Doub, Licensing Program Managers (LPM), Lisa Rios and Liza King, Licensing Program Analysts (LPAs), Arielle Pascua, Noel Wolf-Petersen and Melina Oropeza, Valley Mountain Regional Center (VMRC) Community Services Director, Brian Bennett, VMRC Quality Assurance Division Manager, Katina Richison, VMRC Liaisons, Deanna Fauntlin and Angela Conti, Licensee, Iboni James, Facility Administrator, Nicolette Taylor and Representative for the Licensee, Jacob Reinhardt. The Non-Compliance Conference process was explained during this meeting to include the administrative process.
