Facility Evaluation Report
Licensing Program Analyst Noel Wolf Petersen arrived unannounced to the facility 3/12/2026, 8:45am, to perform a quarterly visit related to a informal meeting regarding a high volume of complaints on observations made while investigating them. LPA met with Staff Nadia Falconer and then later Licensee/administrator Iboni James by phone to explain the purpose of the visit. LPA notes James disagrees with being on a quarterly review schedule, on the basis of not being informed of a quarterly review schedule at the informal meeting and finds it unnecessary.
The LPA checked the facilities guardian roster, and administrator hours against the LIC 500. Administrator has Night Shifts through the week totaling: 20 hours per week. LPA gave guidance this should increase or be staggered in time to assess employee performance and emergency readiness periodically. 4 employees on the roster are guardian cleared, LPA gave guidance that the guardian looks a little cluttered with 44 associated members and that if she would remove any employees that have since left the organization that would be appreciated.
The LPA checked for child locks present on the doors and other obstructions to the doors operation. Child Locks appear to be removed, there are Lifting bar type locks on the sliders to the back yard and a deadbolt on the front door.
LPA checked the database number for the landline in the facility. current landline phone number is the database number.
