Facility Evaluation Report
An informal meeting was conducted today on 11/26/25 at the Sacramento Regional Office via Microsoft Teams. The purpose of this informal meeting was to discuss the high volume of complaints and the observations the department has made while investigating them. Present at this meeting were Licensing Program Manager (LPM), Lisa Rios, Licensing Program Analyst (LPA), Arielle Pascua, Designated Facility Administrator/Licensee, Iboni James and Attorney for facility, Jake Reinhardt.
The facilities have been licensed since 06/03/2025 and 08/31/2025.
LPM Lisa Rios explained the Informal Conference process which included the Administrative Process as well.
The focus of concerns at this time were as follows:
4. Administrator qualification and duties
On 10/9/25 the Department substantiated that an uncleared staff was present at the facility. On 10/20/25 the Department substantiated that a staff verbally and physically assaulted a resident in care. On 11/03/25 the department substantiated a complaint at Dream Care #1 that residents are not able to exit the facility freely by observing a child lock placed on the door making it difficult for residents to open.
