Facility Evaluation Report

On 04/30/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived announced to this facility to conduct a Pre-Licensing Visit. LPA met with applicant, Iboni James, and explained the purpose of the visit.

The purpose of the visit of to follow up on the Pre-Licensing visit that was conducted on 04/02/2025.

On 04/02/2025, it was found that the facility sketch was incomplete and did not reflect the proper facility layout. In addition, a fire clearance was requested to reflect the additional office room.

As of this date, 04/30/2025, the facility has completed the facility sketch and fire clearance to ensure compliance with Title 22 regulations.

The applicant has passed the pre-licensing component of the application process. LPA will notify the Central Application Bureau (CAB) that the pre-licensing has been completed and passed.

An exit interview was conducted, and a copy of this report was given to the applicant.
