Facility Evaluation Report
On 09/23/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived at facility unannounced to conduct a health and safety check related to a resident that was relocated as a result of a Temporary Suspension Order from a prior facility. LPA Pascua met with Staff Member (SM) Sean So and explained the purpose of today's visit. LPA Pascua asked SM So to contact the Facility Administrator to inform them that CCL was present. Shortly after, LPA Pascua met with Facility Designated Representative (FDR), Niscy Abella and Stan Osuagwu.
Current census was 4. A brief interview with FDR Abella was conducted.
LPA Pascua toured the facility including the grounds, kitchen, and bathroom. The facility was observed to have a 2 day supply of perishable food and 7 day supply. The medication was observed to be locked and inaccessible to residents in care. The Medication Administration Record was adequately documented. Resident 1 (R1) was observed to be content. LPA conducted a tour of R1's bedroom which contained all of R1's belongings and furniture was in compliance and in good repair.
Based on the observations made during this visit there are no deficiencies being cited. An exit interview was conducted and a copy of this report was provided to the facility at the end of this visit.
