Facility Evaluation Report
On 09/30/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the community to conduct a post-licensing visit. LPA Campbell met with Caregiver Melissa Riley , introduced themselves and explained the purpose of the visit. LPA Campbell was allowed to enter the community.
The community is licensed to serve 4 ambulatory residents who are 18 to 59 and over and developmentally disabled. It is a two story building . Currently, three residents reside in the community. One resident is out at Day Program. The other two are in the community, watching TV in the living room and in their room.
The kitchen was accessible to residents and clean and sanitary. The LPA observed 7 days of non-perishable and 2 days of perishable food supplies. There is enough clean plates, cups and bowls and cutlery to meet capacity. Staff were observed assisting residents with activities of daily living (ADLs), engaging with resident in care. Residents were observed, watching television or laying on the couch.
The water temperature in the bathroom was measured at 120 degrees F within regulation between 105 and 120 degrees Fahrenheit (F) at faucets accessible to residents. The thermostat was set at 73 degrees F which is between 68 degrees F (26 degrees C) and 85 degrees F (30 degrees C) as required. Window screens were observed to be in good repair.
