Facility Evaluation Report

On 11/19/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility to conduct an annual inspection. LPA Campbell met with Jeevan Sandhu, Licensee and explained the purpose of the visit. Two residents were out in the community and 2 residents are currently present in the facility.

The facility is a two story building that is licensed to serve 4 ambulatory residents who are 18 to 59 and developmentally disabled. Currently, four residents reside in the community, one of whom is present temporarily as a respite.

The water temperature in the bathroom was measured at 120 degrees Fahrenheit (F) within regulation at faucets accessible to residents. The thermostat was set at 73 degrees F During a tour of the outside space, LPA Campbell observed that window screens were observed to be intact and in good condition.

During the course of the visit, LPA Campbell followed up on several issues that had resulted in citations during a prior inspection regarding documentation, fire extinguisher inspections and resident accessible toxins. LPA Campbell requested R1's file and found that they now had a Physician's Report (LIC602) that was completed and signed on 10/16/2025. The fire extinguisher was found to have last been inspected on 10/02/2025 and was fully charged. LPA Campbell inspected the laundry room and found that cleaning products were no longer accessible to residents. Instead, LPA Campbell observed toilet and facial tissue. Cleaning products had been moved to a small locked closet attached to the laundry room.

Of the 16 staff, LPA Campbell reviewed 4 of their files and they were found to be complete and the staff fingerprint cleared. At this time, there are no deficiencies to be cited. An exit interview was held and a report left with licensee Jeevan Sandhu.
