Facility Evaluation Report
On 10/14/2025, Licensing Program Analyst Renee Campbell arrived unannounced to conduct an annual inspection. LPA Campbell met with Christopher Hernandez, Administrator and explained the purpose of the visit. Upon entry to the community, LPA Campbell observed residents eating snacks in the dining room, watching television and socializing. Staff were observed cleaning the bathrooms, doing laundry or working in the kitchen.
The facility is a one story building with 6 bathrooms for residents and staff. There are 15 bedrooms, 8 of which house 2 residents each. The facility is licensed to serve 24 mentally disabled adults ages 18 to 59. Four residents may be non-ambulatory.
LPA Campbell toured the facility and inspected common areas including the kitchen, dining room, television room and bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 68 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured at 111 degrees Fahrenheit, which is within the required range of 105 and 120 degree Fahrenheit. The backyard was toured by LPA Campbell and pathways and exits were found to be clear. A soda machine was observed in the designated smoking area .
Of the 24 residents, LPA Campbell reviewed 3 resident files. Of the 18 staff, LPA Campbell reviewed 2 staff files.
