Facility Evaluation Report
Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to the facility on 10/19/2023 at 8:45 AM to conduct an annual inspection visit. LPA met with Jessica Sanchez, Administrator and explained the purpose of the visit.
The facility is licensed for a capacity of 20 ambulatory residents of which 4 may be non-ambulatory. LPA Campbell observed 4 residents sitting in the patio/dining area watching TV. There was 1 resident in day program and 10 residents who later went on a lunch outing around 12 noon during this visit. LPA toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents. Residents were observed in their rooms, or in the TV/Lounge area. LPA observed kitchen, dining area, bedrooms and bathrooms, storage areas, laundry and lighting throughout the facility. LPA observed supplies of nonperishable foods for a minimum of one week and perishable foods for a minimum of two days maintained on the premises.
The temperature inside the facility was observed to be at 71*F which is within the required range of 68-85 degrees Fahrenheit. The hot water temperature was measure at 107.5 and 108.5 degrees Fahrenheit. in 2 of the 7 bathrooms in the facility which is within the required range of 105-120 degrees Fahrenheit.. LPA observed fire extinguisher(s) were last inspected on 08/09/2023. The smoke and carbon monoxide detectors were tested and there is central heating and air in the facility. LPA observed the centrally stored medications area to be locked and inaccessible to residents. The first aid kit contained the required items such as sterile dressings, bandages, adhesive tape, scissors, tweezers, thermometers, antiseptic solution and guide. LPA Campbell observed toxins, and sharp knives were kept locked and inaccessible to clients. LPA Campbell checked medication storage and found medication to be locked away and inaccessible to clients.
