Facility Evaluation Report
On 08/19/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the community to conduct an annual inspection. Upon entry to the community, LPA Campbell observed residents lining up for medication and eating breakfast in the dining room. A staff member guided LPA Campbell to a visitor room where they could work. LPA Campbell met with Josiah Nicklas, House Manager and explained the purpose of the visit. After discussing the documentation that would be needed, LPA Campbell toured the community rooms, kitchen, bathrooms, bedrooms, outside areas and and the file room.
The facility is a single story building licensed to served 37 ambulatory and 12 non-ambulatory residents between the ages of 18 and 59 years of age who are developmentally or mentally disabled. Rooms are shared between two residents and two rooms shared one bathroom.Once LPA Campbell completed the community tour and selected random files from the file room for review and the report opened, 4 resident files were reviewed. Due to a technical problem, the entries were lost and could not be re-entered due to time constraints but all files were found to be complete. Six staff files were reviewed and also found to be complete.
During the tour, a fully-charged and up-to-date fire extinguisher, and carbon monoxide/smoke detectors were observed. LPA Campbell reviewed a fire drill log for the past year that occurred once a month. The fire extinguishers were found to have last been inspected on 08/05/2025, LPA Campbell observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Campbell observed locked closets and cabinets for the storage of medication. LPA Campbell observed locked storage areas for the storage of cleaning solutions and knives as well. Water temperature was measured at 115 degrees Fahrenheit and the thermostat was set at 74 degrees Fahrenheit.
