Facility Evaluation Report
Licensing Program Analyst (LPA) Jason Lund arrived at the facility unannounced to conduct a one- year required visit. LPA met with Staff and later with House manager Stephanie Senicero and explained the reason for the visit. Census: 4
LPA Lund & House manager Stephanie Senicero toured/inspected the facility is a home with 4 resident bedrooms and 3 bathrooms, with 3 working toilets and 2 showers. Facility has common area, dining area, outdoor area, laundry room and kitchen. All bedrooms have the proper furnishings for all 4 clients in care. The facility has adequate supply of 7-day non-perishable and 2- day perishables stored in the kitchen and pantry. Smoke alarms and 2 Carbon monoxide detectors were observed operational. Fire extinguisher (12/1/2025) were charged, mounted and in compliance. Toxins were locked and stored in the laundry room. LPA Lund reviewed two staff and two client files and are in compliance at this time. Medications are centrally stored and in compliance.
An exit interview was conducted, and a copy report was left.
