Facility Evaluation Report
Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct a 1 year annual inspection. LPA Campbell met with Mary Velazquez, Director and Administrator and informed her of the reason for the visit. The program is open Monday - Friday, 9 am to 3 pm. The current census is 98 clients who consistently attend the program in person and 61 staff and the room temperature was 70 degrees F which is between a minimum of 68 degrees F (20 degrees C) and a maximum of 85 degrees F (30 degrees C).
LPA inspected the facility which included the break room, multipurpose room, classrooms and bathrooms. The client participants bring their own lunches and snacks and the facility has an emergency supply of food for up to seven days. Sharp knives are locked away. The first aid kit was present and complete. There's sufficient lighting throughout the facility. The required fire extinguisher was present and expires 12/27/2024. The day program's last fire drill conducted was 01/09/2024 during today's visit. The program has a pull down fire alarm system which contacts the fire department. LPA Campbell tested the hot water in the kitchen. It measured 107 degrees F. which is in the range of 105-120 degrees F. There are no bodies of water on the premises. LPA Campbell observed indoor/outdoor passageways are free from obstruction.
The Day Program is clean and in good repair. It is a free standing one floor building. There is an outdoor seating area equipped for outdoor use. The multipurpose room is filled with games and learning activities appropriate for clients in care. LPA observed all clients participating in today's activities.
LPA Campbell reviewed the MAR for eight clients and found them to be complete. LPA reviewed 5 staff files, which include fingerprint clearances, Health Screenings and CPR Certificates. A review of staff records indicates that facility staff are fingerprint cleared and associated to the facility
As a result of this inspection, no deficiencies were cited, per Title 22 Regulations. Exit interview was conducted and a copy of this report was given to Administrator/Director Mary Velazquez.
