Facility Evaluation Report

Licensing Program Analyst (LPA) Jason Lund arrived unannounced to conduct one year required annual visit. LPA Lund met with Program Coordinator Anita Mishra explained the purpose of the visit. Census: 90

LPA Lund & Program Coordinator Anita Mishra toured/inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms, resident bathrooms, medication room, laundry room, activity room, and outside courtyards. LPA observed sufficient furniture and lighting throughout the facility. There are no bodies of water present in the facility. LPA observed sufficient seven- day non-perishable and two- day perishable food supplies. Hot water temperature was measured at 109.5 degrees Fahrenheit in resident bathroom sink, which is within the required range of 105 to 120 degrees.

Fire extinguishers (2/12/2025) and smoke detectors are current and in compliance with fire safety. LPA observed centrally stored medications are kept locked and inaccessible to residents. LPA reviewed 8 residents and 5 staff files, including criminal record clearances. The reviewed filles were in compliance. The first aid kit was checked and in compliance.

No deficiencies were observed or cited during this visit. Exit interview held and a copy of report given.
