LPA Jason Lund received an Unusual Incident/Injury Report (LIC 624) on 12/23/2025 from client (C1) left the facility and signed out. C1 had approval from Stanislaus County social worker to go out of the facility. C1 did return to the facility on 12/23/2025.
No deficiencies were observed and cited during this visit.
