Facility Evaluation Report

On 04/09/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA Pascua met with Licensee, Augusta Nkwocha and explained the purpose of the visit.

The purpose of the visit was to follow up on an incident report sent on 03/13/2026 stating that a resident received a medical condition that needed a restricted health care plan.

Furthermore, it was stated that the facility was unable to obtain a follow up appointment with the physician regarding this condition and had to be sent to the hospital due to an infection.

LPA Pascua reviewed the facilities restricted health care plan to ensure it meets the guidelines of Title 22 regulations. The restricted health care plan was conducted on 03/16/2026 highlighting the condition, physicians reports and training conducted.

It was also learned through interviews that the resident was taught how to clean the area to mitigate infection in this area by the physician.

Based on the information gathered, there are no deficiencies based on the information gathered there are no deficiencies cite during this visit.

An exit interview was conducted and a copy of this report was provided.
