Facility Evaluation Report

On 09/23/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA met with Licensee, Augusta Onyema and explained the purpose of the visit.

Current census was 4. 2 out 4 are out of the facility at this time.

The purpose of the visit was to conduct a case management visit to follow up on an incident that was received by the department on 08/28/2025.

It was stated in the incident that R1 was aggressive towards staff and threw furniture such as chairs towards staff while other residents were around. It was stated by staff that they were able to de-escalate resident from continuing to throw items and redirect the residents to their room.

An interview with License stated that the facility has continuously been working with the behaviorist as well as additional training has been conducted to help mitigate behaviors and ensure the health and safety of the other residents at this time.

The department will follow up at a later date if needed.

Based on the observations made during this visit no deficiencies being cited.
