Facility Evaluation Report
On 11/04/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility unannounced to conduct a case management visit. LPA Campbell met with Tanya Monge, Program Director and explained the purpose of the visit.
The current census for today was 27. There were currently 12 staff present.
The purpose of this visit is to follow up on deficiencies cited on 12/19/24 regarding client files that were unsecure and accessible.. The licensee appealed the initial deficiency from 12/19/24. After a review by the department of licensing reports, documentation and investigated information, the appeal was granted due to a technical error. Instead, it was found that the licensee should have been cited for regulation 87070(c)(1)(2) instead.
LPA Campbell discussed with Program Director Tanya Monge the Plan of Correction (POC) put in place to address the issue. An exit intervew was conducted with staff and a copy of the LIC809 and 809-D page was left at the facility.
