Facility Evaluation Report
On 05/12/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to the facility to conduct a case management visit.
LPA Pascua met with Licensee, Augusta Nkwocha and explained the purpose of the visit.
The purpose of this visit was to follow up with an incident report that stated that a resident fell at the facility while going out to the doctor's appointment.
During this visit it was learned that the facility licensee and the facility staff have encouraged the use of the walker for support as needed. However, it was learned that the resident has been refusing to use the walker stating that they do not need it. LPA Pascua verified with the Licensee that an update to the service coordinator regarding consistent refusal of the use of the walker.
LPA Pascua reviewed the resident's IPP as well as other supporting documents.
Based on the information gathered during today's visit there are no deficiencies cited during today's visit.
An exit interview was conducted and copy of this report was provided to the facility.
