Facility Evaluation Report
On 11/20/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA was greeted by Licensee, Augusta Nkwocha and explained the purpose of the visit.
Current census was 4. A brief interview with Licensee Nkwocha was conducted.
The purpose of the visit was to follow up on two incident reports regarding R1.
On 11/09/2025, the department received an incident report for R1. Based on the incident report, R1 reported that they were having a medical emergency. 911 was called and emergency services came to the facility. LPA Pascua reviewed incident report and provided technical assistance to the licensee regarding follow up steps on what the facility will be doing moving forward in assisting the resident.
This documentation will be sent to the LPA and a copy will be put in the facility records.
An exit interview was conducted and a copy of this report will be provided to the facility at the end of this visit.
