Facility Evaluation Report

Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct an annual inspection. LPA Campbell met with Tyrone Penny, Administrator and explained the purpose of the visit.

The facility is licensed for 4 developmentally disabled ambulatory residents between the ages of 18 and 59. The building consists of one story with 4 bedrooms and 2 bathrooms. Per staff, there are 4 residents living at the facility, 2 of whom are currently present. The other 2 were at day program. Of the 4 people on staff, all were fingerprint cleared and their files were complete. Of the four residents, all files were reviewed and found to be complete.

LPA Campbell toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 73 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature is measured 117 degrees Fahrenheit, which is within the required range of 105 and 120 degrees.
