Facility Evaluation Report
Unannounced annual licensing visit made out to this facility on 07/19/2023 by Licensing Program Analyst (LPA) Charlie Yang who was unable to make any contact with facility staff at this time.
The facility was observed to not have residents or occupants at this time. The shades were drawn in every window.
This LPA, after a brief walk around of the exterior grounds, observed that the gates were all latched and locked at this time.
A telephone call was made by this LPA to attempt to contact the facility Licensee, Ramendra Prasad, to inform him that CCL was present at this time to conduct this annual visit.
This LPA was able to reach the facility Licensee, Ramendra Prasad, to inform him that CCL was present and inquired about the current status of this facility.
The facility Licensee, Ramendra Prasad, stated that this facility has been vacant and void of any staff and residents since the initial Prelicensing visit that was conducted on 07/21/2022. There have not been any changes made to the physical plant or program since that time.
The facility Licensee, Ramendra Prasad, stated that this facility was waiting for the completion of the facility program through Valley Mountain Regional Center in order to attain vendorization and start accepting residents in care. Until that time, this facility will remain closed and unoccupied.
The facility Licensee, Ramendra Prasad, stated that he was unaware of a time frame when this facility will be able to start accepting residents into care. In the meantime, this facility will remain closed until the first resident is accepted.
