Facility Evaluation Report
On 08/13/2025, Licensing Program Analyst (LPA) Renee Campbell arrived to the facility to conduct an annual inspection. Upon entry, LPA Campbell observed residents in the dining room eating and socializing. LPA Campbell met with Staff Janie Wooding and explained the purpose of the visit.
During the tour of the community, LPA Campbell observed the kitchen, two bedrooms, outdoor space and common areas. In the kitchen, a refrigerator was observed with enough perishable food items to last two days. The refrigerator temperature was observed at 36 degrees Fahrenheit (F). Enough Dry good food items were observed in the food pantry to last the community 7 days.
The two bedrooms observed, contained a bed, chairs, a chest of drawers and a closet. Residents were observed in the common area watching TV. Appropriate furniture suitable for the community was observed. Outdoor space in the backyard was green and shaded. Additional food storage freezers were observed outside and locked due to prior theft from outside of the community. The freezer and refrigerator inside are unlocked and accessible to residents.
Toxins, medications and equipment were observed to be locked and inaccessible to residents. LPA Campbell observed a Mental Health Patients Rights poster and another poster displayed the Ombudsman's number. The Type A fire extinguisher was last inspected on 09/25/2024. LPA Campbell reviewed 3 residents files and 2 staff files. All files were found to be complete.
