Facility Evaluation Report
On 07/09/2025, LPA Campbell arrived to the facility to conduct a case management. LPA Campbell met with Joan Little and explained the purpose of the visit.
On 04/21/2025, LPA Campbell interviewed S1 regarding an incident on 04/15/2025 when R1 refused to take their medication. On 04/21/2025, S1 reported that R1 had taken their medication as directed on 04/15/2025 earlier in the day and that R1's doctor had stated that R1 could miss a dose. During today's visit, S1 also reported that it was not until R1 had left that they learned that the dose could be missed. However, when R1 refused to take their medication later in the day on 04/15/2025, R1 stated that S1 threatened to "call Doug" if R1 did not take their medication.
Per California Code of Regulations (CCRs) - Title 22, Division 6, Chapter 8, the following deficiency is being cited on the attached 809-D during this visit.
An exit interview was conducted, and copies of the report and appeal rights left.
