The inspector’s account

Licensing Program Analyst (LPA) Renee Campbell arrived at the community unannounced to conduct an annual inspection on 07/24/2025. LPA met with Patricia Lactaoen, Direct Support Provider and explained the purpose of the visit.

Upon entry, LPA Campbell observed staff working on administrative tasks. There were two residents present in the community and one of them was waiting for their ride to day program. LPA Campbell inspected the physical plant including but not limited to the common area, kitchen, dining area, client bedrooms, client bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations . This community is a single story building licensed to serve sixteen (16) ambulatory residents and seven (7) ambulatory among the developmentally disabled.. LPA Campbell observed the facility to be free of odor, clean and in good repair. LPA Campbell observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present . The furniture for one of the empty rooms had been removed so that the flooring could be changed out while 15 of the residents are away at camp.

LPA Campbell observed sufficient seven-day non-perishable and two-day perishable food supplies. Hot water temperature was measured in the kitchen and bathroom. The kitchen water temperature was measured at 108 degrees Fahrenheit (F) and the resident bathroom water temperature was measured at 108 degrees F. Fire extinguishers were observed and, smoke and carbon monoxide detectors were tested and are in good repair. The fire extinguisher was last inspected on 11/25/2024. The Facility thermostat was set at 71 degrees F. LPA Campbell checked medication storage and found medication to be locked away and inaccessible to clients in a cabinet. The first aid kit was complete and contained scissors, tweezers, manual and thermometer.
