Facility Evaluation Report

Licensing program Analyst, Noel Wolf Petersen Arrived unannouced to the facility to conduct a case management related to eviction procedure. LPA met with Administrator Heather McClosky, to explain the purpose of the visit.

LPA went over the eviction procedures with the administrator

Licensing addressed any expressed concerns with eviction procedures with the clients. a copy of the report was read and given to the administrator. no citations were associated with this visit. exit interview was conducted.
