Facility Evaluation Report
At approximately 9:00 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced at the facility to conduct a visit in conjunction with a Code Inspector for Sonoma County regarding facility deficiencies. LPA was informed that the facility’s clients have been relocated
During the visit LPA reviewed outstanding deficiencies that need to be cleared with facility Administrator Marylis Gachu. LPA provided a copy of an email that was sent to the facility on 10/8/2025.
No deficiencies cited during today's visit.
