Facility Evaluation Report

Licensing Regional Manager – Carla Nuti-Martinez, Licensing Program Manager – Victoria Bertozzi, Licensing Program Analyst – Robert Frank, Katy Vanzant-Quality Assurance Supervisor, Meghan Dill-Quality Assurance Monitor (via Teams Meeting) met with Licensees Debbie & Robert Miller to address areas of non-compliance.

On July 11, 2025, the Department received a complaint alleging staff members were violating a client’s Personal Rights. Based on the complaint investigation the Department substantiated the allegation.

On November 18, 2025, the Department received a complaint alleging neglect and lack of supervision of a client which led to a facility dog biting the client. Based on the complaint investigation the Department substantiated the allegation.

The following areas of non-compliance were addressed today:

-Communication with facility staff of changes in client behavioral issues or areas of concern.

Licensee has agreed to be a part of the Technical Support Program.
