Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct a case management visit to follow up on a telephone call received on 1/29/26 from Licensee of the facility, Emilce Deller regarding a change of ownership. Upon LPA arrival, staff granted access to the facility. Licensee was not able to come to the facility due to medical reasons, but was available by phone and gave authorization for staff to sign the report.

On 1/29/26, The Licensee contacted LPA via phone to notify the Department that the facility is under a Limited Liability Corporation (LLC). According to the Licensee, they didn't remember that they have switched to a limited liability company (LLC) since 2020, and Emilce Deller will be the Licensee in LLC of this facility. During today's visit, LPA learned that current (LIC309) Administrative Organization form indicates that this facility is in a sole proprietorship of two Licensees. As of today’s date, a new application has not been submitted to the Department. License acknowledges that they are responsible for the operation until the new application is approved and issued. The Licensee agrees to provide documentation to ensure control of property. The Licensee confirms understanding that the current license is not transferable and an application is required to be submitted to the Department for change of ownership. The Licensee agreed to submit a change of ownership application with new corporation or LLC to the Centralized Application Bureau (CAB) for the facility by not later than April 10, 2026.

Deficiencies are cited from the California Code of Regulations (CCRs), Title 22, Division 6, Chapter 8 and the Health and Safety Code. Failure to correct the deficiency and/or repeat deficiencies within a 12-month period may result in civil penalties. Appeal Rights Given. Exit interview conducted with staff and a copy of this report was given.
