Facility Evaluation Report
Licensing Program Analyst (LPA) Marisol Cuadra arrived unannounced to conduct a Case Management - Incident Visit and met with Licensee Anton Kuehnhackl. The purpose of today's visit is to follow up on an incident report received at CCL on 6/15/26.
T he report states that on 06/14/2026 at approximately 4:03pm, client (C1) fell out of bed, landed on their right side, injuring their cheekbone and temple. According to licensee, 10 minutes prior they had just checked them before washing dishes to reposition them, C1 was elevated in bed slightly, but not completely up when suddenly heard a loud bang from C1's room, ran over and found C1 on the floor face down, on their right side, after getting C1 back in their bed carefully, licensee assessed C1 for injuries, noted swelling forming on their right cheekbone and upper temple, a small cut on their upper right temple with a size about of one centimeter, but no visible bleeding, neck or head injuries was observed. Licensee stated that they treated their wounds with ice on and off every 15 minutes along with Arnica Gel. C1 was observed alert after the fall, despite being in pain and was able to swallow, chew, and operate their facial extremities normally. C1 was given over the counter Tylenol later in the night to help with pain and swelling. The facility will follow up with C1's Primary MD regarding injury.
