Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived at facility unannounced to conduct a case management was greeted by Administrator Ernesto Idang.
During today's visit, LPA explained to the Administrator that there is a second notice letter sent to them after unsuccessful attempts via mail on 9/15/25, regarding their outstanding fees in the amount of $742 indicating that if we do not receive evidence that your fees have been paid by 9/23/25, the department may initiate legal action to close the facility. Administrator submitted online payment and provided proof that their fees are current. Also, LPA suggested to the Administrator to review and submit any update needed for their facility contact information including their phone number to the Department.
No deficiencies cited during today's inspection.
