The inspector’s account

Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct an Annual Required inspection and met with Licensee Kenne Kennemer. Clients were attending to day program during the visit.

LPA/Licensee initiated a tour of the facility at 12:00 pm and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Client rooms were furnished per regulation. Water temperature in client's bathroom measured at 110.4 degrees F which is within allowable range of 105 to 120 degrees F. Extra hygiene products and linens were available. Cleaning supplies were locked in a bathroom closet and under the kitchen sink. Facility has at least two days of perishable and one week of non-perishable foods. Medications were centrally stored and locked in a hallway closet along with client cash resources. Cash resources and their records were reviewed. Smoke detectors were tested and operational. Fire extinguisher was last inspected February, 2026. Required postings were observed.

File review was initiated at 12:45 pm. Three staff files and three client files were reviewed. Staff have required First Aid certificates and training hours required. Administrator Certificate for Licensee, Ken Kennemer, 7034413735, expires on 5/14/2026. Medications and medication records were reviewed, but one out of three clients do not have expiration date entered into the Centrally Stored Medication Logs (Technical violation issued). During today's visit, LPA have a conversation with the Licensee regarding the possibility of submitting an exception to retain clients who are 60 years of age, but today LPA was informed that there are clients who are in the process of been relocated to a different facility due to their needs are higher. LPA encouraged Licensee to submit an exception timely in case that they needed it. Licensee provided copies of the following documents: LIC 500 Personnel Summary and surety bond. No deficiencies cited during today's visit. Exit interview conducted with Licensee and a copy of this report was given.
