Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct an Annual Required inspection met with Licensee, Lisa Melo. Required postings were observed. Annual fees are current.
LPA/Licensee initiated a tour of the facility and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Residents rooms were furnished per regulation. Extra hygiene and linens were available. Facility has at least two days of perishable and one week of non-perishable foods that appeared to be stored per regulation. Hot water measured in bathrooms at 106.3, 108.2 and 105.1 degrees F which are within the range of 105 to 120 degrees F allowed per regulation. Fire extinguisher was last serviced July 2026. Smoke detectors and carbon monoxide detectors were tested and operational. Last disaster drill conducted on June 13, 2026. Disinfectants and cleaning solutions were stored inaccessible to residents.
LPA initiated file review at 9:30 am. LPA reviewed five residents files and three staff files. All residents have their medical assessments and care plans updated. Staff records have current First Aid/CPR certificates and additional 20 hours of required training. Medications and medication records were reviewed. Administrator Certificate for Lisa Melo, 7034467740, expires on 11/16/2026.
Licensee submitted updates of the following documents: Designation of Administrative Responsibility (LIC308), Personnel Report (LIC500) and copy of Liability Insurance.
No deficiencies were issued during this visit. Exit interview was conducted with Licensee and a copy of this report was provided.
