Facility Evaluation Report

Licensing Program Analyst Leibert, during the course of complaint investigation, noted that 4 resident apartments in the facility had electrical problems involving the ceiling lighting fixtures that were not operating. Management indicated a request for authorization for the electrical repairs has been made to the Home Office but has not yet been approved. Lighting has been inoperable for at least two weeks.

The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided.
