While at facility delivering complaint findings, LPA Hansen conducted case management to address issues revealed during complaint investigation. Complaint (21-AS-20250521082833) revealed staff (S1) witnessed alleged abuse of resident (R1). Staff failed to notify of alleged abuse in required time frame. Facility agrees to do all staff training to review mandated reporting and facility policies and procedures.
