Facility Evaluation Report

Licensing Program Analyst Leibert arrived unannounced for the purpose of amending a citation issued on 12/31/24. In addition, a civil penalty in the amount of $400.00 for failure to correct was issued. The front door does not lock and, as of today, has not been repaired. Administration has been advised that additional penalty in the amount of $100. a day will be assessed until the deficiency is corrected.
