The inspector’s account

At approximately 8:40 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Case Management visit and was greeted by Assistant House Manager (AHM) Nahashon Kigathi. Utulivu Home is Licensed as an Adult Residential Facility - Enhanced Behavioral Supports Home (ARF-EBSH). The facility has an approved fire clearance for three (3) non-ambulatory Clients between the ages of eighteen (18) and fifty-nine (59). Upon arrival, LPA was informed that there were three (3) clients in care.

Today’s visit was conducted to confirm compliance regarding areas of concern noted on a Department of Developmental Services (DDS) inspection on 6/3/2025. The areas of concern noted by DDS are:

-One cupboard in the garage, which is used as a recreation space for the residents, was accessible and contained a small can of paint and a hammer.

-A magnet key was found accessible in the laundry closet, as well as a set of keys found in an accessible in a drawer in the kitchen.

LPA observed all tools and toxic substances in the garage to be secured in locked cabinets/cupboards. LPA further observed that both magnet keys and manual keys were secured on staff members persons or in locked drawers.

No deficiencies cited during today's visit.

Exit interview conducted. Copy of report and discussed and provided to AHM Kigathi. Signature on form confirms receipt of documents.
