Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct an Annual Required Inspection and met with Vernada Townsend , staff. Administrator, Raquel Gomes was not able to come but she was available by phone and gave authorization for staff to sign the report.
LPA/staff initiated a tour of the facility and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Client rooms were furnished per regulation. Water temperature in client bathrooms measured at 115.3 and 115.7 degrees F which are within regulations. Extra hygiene products and linens were available. Cabinets containing cleaning supplies located in the garage were locked. Facility has at least two days of perishable foods and one week of non-perishable foods. Medications were centrally stored and locked. Fire extinguisher was last inspected August, 2025. Smoke detectors are hardwired and maintained by a vendor. Most recent service was conducted August, 2025. Carbon monoxide detector was tested and operational. Most recent fire drill was conducted June 6, 2026. Exit Auditory alarms were on and working during visit. Medications and medication records were reviewed. Residents rooms are located downstairs and they do not go upstairs, all residents are ambulatory, no evacuation chair is needed. Cash Resources were reviewed. Required postings were observed. Clients are attending to day program.
