Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra conducted a case management visit to cite deficiencies discovered during a complaint investigation #21-AS-20220202091317 and met with Licensee, Sarah Lawrence and Administrator, Megan Lawrence-Ferreira.
LPA learned through records review that supplemental staffing request dated 6/11/21 was approved by North Bay Regional Center indicating that C1 required one on one supervision to provide awake overnight supervision due to behavior challenges, supplemental staffing was approved for 15 hours per day effective 1/1/22 to 1/31/23, but investigation revealed that facility only provided overnight supervision for two to three nights per week.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12-month period may result in civil penalties.
